Here are Clallam County Environmental Health Services’ restaurant/ food businesses’ inspections for the month of August.
Inspectors visited 17 establishments, and found seven locations with perfect scores. One was a reinspection.
Ten locations were found with varying health and/or retail practice violations. Some establishments corrected measures during the initial inspection. Four require follow-up inspections.
The following establishments received perfect scores:
• Angeles Acai, 305 Kreaman Road, Port Angeles
• Galileo’s Mobile Pizzeria, 2221 Mountain View Ave., West University Place
• Hurricane Coffee Company, 104 W. Washington St., Sequim
• Just Rewards Espresso, 1001 E. First St., Port Angeles
• Longhouse Cafe, 81 N. Forks Ave., Forks
• Pacific Pantry, 229 S. Sequim Ave., Sequim *
• Reddog Coffee, 521 S. Sequim Ave., Sequim

The following establishments were determined to have a less-than perfect score:
• Black Bear Diner, 1471 E. Washington St., Sequim
Violation (8-point deduction): a sanitizer test kit not being used to ensure chemical sanitizing solutions are at the proper concentration; nonfood contact surface of the cookline has accumulations of grime and debris. Correct by Aug. 13. No follow-up inspection required.
• Koto Teriyaki and Sushi, 1252 W. Washington St., Sequim
Violation (10-point deduction): person in charge needs to renew Certified Food Protection Manager certification; reduce fly presence in kitchen and throughout facility. Correct by Aug. 22 and 29. No follow-up required.
• Moon Palace Restaurant, 323 E. Washington St., Sequim
Violation (10-point deduction): obtain certified food protection manager certification; keep bulk food storage equipment clean and sanitized; repair chipping paint on ceiling food storage area. Correct by Dec. 15 and 29. No follow-up required.
• David Family Taqueria, 300 E. Washington St., Sequim
Violation (13-point deduction): person in charge did not have a valid food worker card; food temperatures read 42-45 degrees and must be kept at 41 degrees or lower; thermometers not in refrigerators. Correct by Aug. 21. No follow-up required.
• Viking Ice Cream, 20373 Viking Ave., Poulsbo
Violation (13-point deduction): handwash sink is being used for purposes other than washing hands; in-use utensils improperly stored between use. Both were corrected during inspection. No follow-up required.
• Sequim Kabob House, 173 W. Washington St.
Violation (15-point deduction): Certified Food Protection Manager classification needed; keep facility in proper operation according to health code; handwashing facilities unclean and not maintained (corrected during inspection). Obtain certification by Sept. 4. Follow-up inspection required.
• Oasis Bar and Grill, 301 E. Washington St., Sequim
Violation (18-point deduction): person in charge needs a valid food worker card; soap not provided at handwashing sink in bar (corrected during inspection); a sanitizer test kit not being used to ensure chemical sanitizing solutions are at the proper concentration. Remaining corrections by Aug. 12. No follow-up required.
• Sequim Bento Teriyaki, 1243 W. Washington St., Sequim
Violation (23-point deduction): obtain Certified Food Protection Manager certification; one person was missing or had an expired food workers card; food read a temperature of 55 degrees with proper cold holding at 41 degrees or below; person in charge could not provide a food thermometer. Correct by Aug. 25 for employee card and Sept. 8 and 9 for other violations. Follow-up inspection required.
• Pacific Pizza, 870 S. Forks Ave., Forks
Violation (23-point deduction): deduction made but no notes about person in charge; food read temperature at 50 degrees and needs to be 41 degrees or below; food-contact surfaces on can opener, mixer were not sanitized; nonfood contact surface of the walk in, floors and halls had accumulations of grime and debris. Correct by Aug. 27 and 29. Follow-up inspection required.
• Mobile 101, 33 E. Taylor Cutoff Road, Sequim
Violation (35-point deduction): obtain Certified Food Protection Manager certification; food temperature was 119 degrees and needed to be 135 degrees (corrected during inspection); thermometers not in all hot and cold holding cases (corrected during inspection). Certification required Dec. 31. Follow-up inspection required.
The health department also inspected six pools and reported four violations.
The following establishments received perfect scores:
• Olympic Inn and Suites, 1510 E. Front St., Port Angeles
• Four Seasons Ranch, 673 E. Strait View Drive, Port Angeles
The following establishments were determined to have violations:
• Forks Motel, 351 S. Forks Ave., Forks
Violation: critical item with disinfectant level high but corrected during inspection; pH low and also corrected during inspection; non-critical items: skimmer not in place; flow meter inoperative. Correct by Aug. 27. No follow-up required
• Olympic Lodge, 140 S. Del Guzzi Drive, Port Angeles
Violation: non-critical item: chemicals improperly stored and locked; pool modifications made without required plan review and approval. Correct by Aug. 22. No follow-up required.
• Red Lion Hotel Port Angeles, 221 N. Lincoln St., Port Angeles
Violation: non-critical item: chemicals improperly stored and locked. Correct by Aug. 22. No follow-up required.
• Salish Trails RV and Campground Park, 80 E. O’Brien Road, Port Angeles
Violation: non-critical item: water clarity is hazy or cloudy with the operator aware and working on a solution. Correct by Aug. 20. No follow-up inspection required.
