Here are Clallam County Environmental Health Services’ inspections for the month of June.
Inspectors visited 35 restaurants and food establishments, and found 21 locations with perfect scores, and 14 locations with varying health and/or retail practice violations. Some establishments corrected measures during the initial inspection, or have been inspected again and received perfect scores, as noted.
The following establishments received perfect scores:

• Costco Wholesale, 955 W. Washington St. (the store and food court had separate inspections)

• Dry Creek Elementary, 25 W. Rife Road, Port Angeles
• Forks Elementary School, 301 S. Elderberry Ave., Forks
• Forks Middle School, 121 Spartan Ave., Forks
• Frugals, 1520 E. Front St., Port Angeles
• Grandview Grocery, 802 W. C St., Port Angeles
• Grocery Outlet, 2823 E. U.S. Highway 101, Port Angeles
• Hamilton Elementary, 1822 W. Seventh St., Port Angeles
• Hardy’s Market, 10200 Old Olympic Highway, Sequim
• Jefferson Elementary, 218 E. 12th St., Port Angeles
• Laird’s Corner Market, 242811 W. Highway 101, Port Angeles
• Neah Bay K-12 School, 3560 Deer St., Neah Bay
• Pies Cakes and More, 1940 E. First St., Suite 156, Port Angeles
• Port Angeles High School, 304 E. Park Ave., Port Angeles
• Rainshadow on Fifth Avenue (inside Olympic Medical Center), 840 N. Fifth Ave., Sequim
• Stevens Middle School, 1139 W. 14th St., Port Angeles
• Warehouse Demo Services, 955 W. Washington St., Sequim
The following establishments were determined to have a less-than perfect score:
• Forks High School Addition, S. Camas Ave., Forks
Violation (2-point deduction): Less than 50 foot-candles of light was noted at the food prep table; staff asked to provide at least 50 foot-candles by Sept. 10, 2025 with no follow-up needed.
• Fresh Wok, 1603 E. Front St., Port Angeles
Violation: (3-point deduction): Sweep and mop walk-in; clean and sanitize bulk food storage lids by July 7 to preclude accumulation of soil.
Follow-up: Restaurant received perfect score at July 1 inspection with inspector writing “Progress being made on general cleaning. Follow up on the next inspection.”
• Carlsborg’s Old Mill Cafe, 721 E. Carlsborg Road, Carlsborg
Violation (3-point deduction): Provide food workers cards onsite for verification; no follow-up needed.
• Chevron Sequim, 23 E. Hooker Road, Sequim
Violation (5-point deduction): Dispose of wastewater into appropriate mop sink; no follow-up needed.
• HWY 101 Food Mart, 1023 E. Front St., Port Angeles
Violation (5-point deduction): Clean and sanitize inside of ice machine, observed mold/dirt accumulation; no follow-up needed.
• The Cracked Bean Coffee Company, 10191 Old Olympic Highway, Sequim
Violation (5-point deduction): Install thermometers in all refrigerator units; no follow-up needed.
• Subway Sandwich Shop, 680 W. Washington St., E101, Sequim
Violations (8-point deduction): Repair leaking plumbing under compartment sink, and men’s restroom; no follow-up needed.
• The Coffee Cottage, 1921 W. Highway 101, Port Angeles
Violations (15-point deduction): Sausage patty temperature at 50 degrees, proper cold holding is 41 degrees or below; No food thermometer available; no follow-up needed.
• Taco Time Sequim, 110 River Road, Sequim
Violations (15-point deduction): No paper towels in the men’s bathroom; chili heated to 165 degrees, needs to be held at 135. Both were corrected during inspection.
• Traylor’s Restaurant and Lounge, 3256 E. Highway 101, Port Angeles
Violations (15-point deduction): Person in charge was unable to demonstrate knowledge of the regulations to the inspector; raw burgers and cooked sausage temperatures read 48-49 degrees.
Follow-up: Restaurant received perfect score at July 1 inspection with inspector writing “refrigerator being repaired and currently maintaining appropriate cold holding temperatures.”
• Applebee’s Neighborhood Grill and Bar, 130 River Road, Sequim
Violations (20-point deduction): Person in charge must keep sanitizing buckets at the proper levels on the cook line; sanitizer bucket measured at 50ppm, below minimum 200-400 ppm required level. Corrected during inspection.
• Pacific Pizza, 870 S. Forks Ave., Forks
Violations (20 points): Kitchen and walk-in need deep cleaning; cutting boards need to be cleaned or replaced; diced tomato and cut lettuce temped at 45 degrees and must be at 41 degrees or below; person in charge needs knowledge of standards by July 25, 2025.
• Corner Restaurant, 2365 E. Highway 101, Port Angeles
Violations (33 points): Obtain certified food protection manager certification; obtain food workers cards for all food handlers; repair soap dispenser at hand sink; thermometers broken.
Corrected: Restaurant received perfect score at July 1 inspection with inspector writing “malfunctioning refrigerator removed; hand wash station properly stocked at the time of inspection; owner stated that his manager will take the certified food protection managers course.
• Fairmount Grocery, 1137 W. Highway 101, Port Angeles
Violations (35 points): Chicken strips and corn dogs were 125 degrees as proper hot holding temperature is 135 degrees; egg salad sandwich in the grab-and-go-case temped at 50 degrees as proper cold holding is 41 degrees or below. A follow-up was marked but no re-inspection had been done as of press deadline.
